COVID-19 SAFETY PROTOCOLS AT ROSENEATH DENTAL PRACTICE

To visit Roseneath:

  • You must be completely asymptomatic (free of fever, cough, altered smell or taste, or breathing difficulties);

Dental care after the lockdown – Dr Michael Nigro, Principal Dentist at Roseneath in Richmond, shares his experience…

Dr Michael Nigro, a Senior Partner at Roseneath Dental Practice, shares his experiences of providing dental care during the lockdown, reopening and the latest developments in the world of dentistry.

Michael, start off by telling our readers about Roseneath:

We are the largest and longest established practice in Richmond, celebrating our 25th anniversary during lockdown! We pride ourselves in providing the full spectrum of both simple and complex dental and orthodontic treatments, always focusing on prevention and treating to the highest standard. The benefits of our approach were evident during the lockdown when the vast majority of our patients required no form of emergency care and the very few who needed help we were able to support without the need for them travelling to local emergency dental centres. Procedures that were already planned during that period had to be cancelled but we were able to reopen on time and those have now all been completed. Our emphasis is on educating our patients about the importance of good oral hygiene and treating potential problems timeously.

Many dental practices complained about being unable to return to dentistry in a timely manner as there was little notice given for the reopening of the dental services:

That is true. As soon as we went into lockdown the team started preparing for the reopening. We took part in online meetings and kept in contact with our colleagues around the world who were coming out of lockdown so we could be prepared for the new protocols. Before any announcements were made our practice was already equipped with all the mandatory requirements to ensure our safety and that of our patients. To improve our patients experience we upgraded our dental software to allow for the elimination of paper forms such as registration forms and medical history questionnaires. Everything now happens electronically, with patients being able to fill in various forms online at their leisure at home, prior to visiting the practice, cutting down on waiting time.

I am aware that Roseneath is very tech-savvy, could you tell us more:

We continuously strive to provide our patients with the best care possible, to do this you have to embrace the latest technology. All our x-rays are digital, which enables us to reduce radiation to a minimum whilst still maintaining the highest image quality. We have the latest in cone-beam CT scanners which provides 3D scans, a must for providing dental implants. Our hygiene sessions are performed according to Guided Biofilm Therapy protocol using state of the art AIRFLOW®, the latest Swiss technology, providing painless efficient plaque removal. We have iTero intraoral scanners, fantastic machines that capture 3D images of the teeth which can then be used to restore teeth using porcelain crowns or onlays, or they can be used in orthodontics eliminating the need for dental impressions. The technology allows us to show patients what they will look like following a procedure before it has been done, particularly useful in orthodontics. Given the current situation, without regular access to preventive dentistry and with ongoing changes to our eating habits, it is now more important than ever to look after your oral health.

In this short video Dr Kieryn Smart, Principal Dentist at Roseneath, shares some tips on how to keep your teeth and gums in good shape during these challenging times.

In making this informal video Kieryn’s aim was to touch base with all the patients of Roseneath and the wider community with the intention of emphasising one’s ORAL HEALTH and with the encouragement to exit this unprecedented lockdown with an invigorated oral hygiene routine. Kieryn also uses the opportunity to keep the audience informed and reassured as to the proactive steps that the practice is taking to, as always continue treating patients in a safe environment putting their safety and that of the staff first.

At Roseneath Dental Care we believe that the introduction of dental implants in Richmond in the early 90s has revolutionised our world of dentistry.

Dental implants are not new to dentistry, with the first dental implant having been placed in 1965 but are now considered a routine dental procedure.

Dental implants are definitely a technologically advanced form of reconstructive dentistry and the definite choice for people unwilling to compromise on their treatment. A dental implant is an alternative way of tooth and mouth reconstruction from routine reconstructions such as removable dentures and bridges.

Here at Roseneath Dental Care, we have 20 years of dental implant experience and as such we have seen firsthand the enormous benefits they have contributed to peoples day to day lives. We have a dedicated and customized implant placement suite and as such routinely offer patients an implant option where indicated during our treatment planning sessions and discussions when formulating a bespoke treatment plan for them.

Dental implants are used routinely to replace single as well as several missing teeth. The main function of the jaw bone is to hold the roots of the teeth. When a tooth is extracted the bone begins to shrink (bone resorption). Implants take the place of tooth roots and give the supporting bone something to do and stop the shrinkage. Another unwanted side effect of a tooth or teeth being extracted is that the surrounding teeth start to move towards the missing tooth area, as well as the teeth opposite the missing tooth gap start to overgrow and become longer and change the bite. Following the loss of teeth, the remaining teeth often become overloaded leading to them loosening, cracking, or being prematurely lost.

Technically the placement of a traditional fixed bridge requires the adjacent teeth to be prepared or ground down into pegs and the restoration spans and involves more teeth than just the missing tooth. As such the dental implant is a great deal more conservative and preserves more tooth structure replacing just the missing tooth.

Dental implants can truly improve the quality of life and enjoyment of eating. At Roseneath Dental Care we aim to keep patients healthy and never need dental restorations. But when there is a need for a tooth or teeth to be replaced we strive to consistently offer our patients the definitive choice of uncompromising treatments with dental implants.

In a recent survey of children’s teeth in the Greater London area, children in Richmond Upon Thames came out with the ‘city’s best rates of “perfect teeth”’, making the Roseneath team of children’s dentists in Richmond very proud and optimistic about the future of children’s dentistry.

Children’s dentistry has and is constantly evolving; now more than ever the emphasis is on prevention. Routine checkups from an early age, dietary advice, protective sealing of the first adult teeth, and children’s hygiene sessions are now considered standard practice.

The bar is consistently being raised and the desired outcome of 100% filling-free teeth is now achievable and becoming the norm at our progressive dental practice.

There are now well-established links between our general health and wellbeing; and the mouth, teeth, and jaws. Much of the emphasis in modern medicine today has shifted to the treatment of the body as a whole and its complex interlinking of systems. This is also very true of dentistry today. Our progressive team at Roseneath Dental Care do just that.

For many years our dentists have been carrying out dental and face developmental checks as part of the routine dental checkups for children. These are incredibly important if we take into account that by the age of 7, about 80% of jaw growth has already occurred. Well-aligned and correct size teeth and jaws play a major role in a child’s development and are the foundation for good health in adulthood.

The dentists at Roseneath Dental Care have actively studied face and jaw developments. With the aid of digital radiographic face and jaw analysis, a dental blueprint of the face and jaws can be produced. This along with the analysis of plaster casts of the teeth and jaws gives us the means by which to predict growth patterns, jaw size, and face profile.

Our aim is to establish a beautiful, lifelong, and well-functioning wide smile on a sound foundation that contributes to good dental and general health.

Early interceptive dental orthopaedic appliances are used to correct jaw, bite, and dental irregularities. The most common presenting signs are crowded teeth (which indicate a small jaw size); irregular bites, snoring at night, mouth breathing, bad breath, speech defects, lisping, and thumb sucking.

Just as parents make lifestyle, health, dietary, and education choices for their children, we strongly believe that dental developmental checks are essential to every child’s wellbeing.

Let’s firstly consider the teeth themselves. If plaque is not allowed to accumulate and spend long periods of time on the tooth surfaces, decay simply cannot develop.

For those who have never had a filling, maintaining a good routine will prevent their unblemished record being ruined. For those of us who have dental work, the incentive is that the work we have will last longer meaning less frequent dental treatments – a good motivator!

Now let’s consider the gums. Here gum disease is the big enemy. Once again a build-up of plaque is the problem. It initially inflames the gums which cause them to bleed. If left undetected it will gradually cause the gums to strip away from the teeth. Unpleasant I know, and not only that teeth can eventually loosen and fall out. If losing teeth was not bad enough, gum disease is also linked to certain other diseases including heart disease. Clearly not an issue to take lightly.

Looking after our teeth and gums is a team effort. Doing our best at home is important. There are however challenges. Firstly, we clean our teeth blindly. By that, we mean that many of the areas we are trying to clean are not actually visible to us. Secondly, we tend to clean our teeth when we are tired, distracted, and even rushed. So even with our best efforts, we are often not totally efficient.

That is why seeing the hygienist on a regular basis is so important. There are dentist Richmond professionals who can get into the areas we are missing and teach us how to improve our cleaning. How often one needs to see a hygienist varies from person to person. Someone with a perfect mouth who maintains a good cleaning routine may only need to go every 6 months, but someone with gum disease or lots of advanced treatments like implants should go at least every 3 months.

And let’s face it, it is a lot cheaper and less unpleasant than many of the problems it’s preventing!

Monitoring the growth and development of a child is essential and should include the development of the face.

Orthopaedic treatment for children

This usually forms part of a child’s routine dental checkups. It is easily done, and it is important to detect problems as early as possible as it is found that the majority of children are not developing correctly!

Some may remember having teeth removed as a youngster in order to make space for the other teeth. Well, that was avoidable had the correct treatment been carried out earlier on.

Children with jaw development problems often have breathing issues amongst others. If not corrected the problems are compounded when one is an adult. Temporomandibular (jaw) joint problems, bite problems, clenching and grinding teeth at night and an unattractive facial profile are all the result of the jaw development not being corrected timeously.

The two main problems encountered in children are jaws that are not the correct size and jaws that are not in the correct position.  Both situations are treatable and the treatment is called ORTHOPAEDICS. The treatment involves the child wearing a simple brace (often removable and colourful!) and is painless. The braces assist the jaws to grow to the correct size and to position them correctly.

The end result is a child that has unobstructed breathing, the correct bite, a happy TM joint, and a normal face profile. And of course no extracting of healthy teeth!!!

Gone are the days when we plugged teeth with toxic metal.

Today we restore simple cavities with neutral composite materials that return teeth to their former glory. The new generation of white fillings comes in different shades so we can match them to the individual tooth colour. The other benefit is that these new fillings are bonded to the tooth so that we don’t get the leakage that was associated with the old metal types.

When a cavity is large and there is not enough tooth to support the filling our treatment of choice is a porcelain onlay. It consists of a restoration that fills the tooth but that also importantly covers the biting surface, giving the tooth added strength. Once again we can match the colour and because there is no metal present the cosmetic result is excellent.

Where possible we prefer to do onlays as opposed to crowns because much of the original tooth is retained. However, in cases where a crown is an ideal choice we now use all-porcelain crowns. Many may have seen or experienced the unsightly dark rim one sees around the old generation porcelain and metal crowns. Well now that is a thing of the past, we can get a perfect match!

Francesca is a Specialist Orthodontist and practices at Roseneath on a part-time basis. She also works as a consultant orthodontist at the Conquest Hospital in East Sussex. She qualified from the Catholic University of Rome, Italy, where she also completed her orthodontic postgraduate training. In 2007 she completed her Ph.D. in orthodontics.

Her level of expertise enables her to successfully treat a wide range of challenging orthodontic cases including patients who need a combined treatment approach with maxillofacial surgery. She is experienced in the latest orthodontic techniques including lingual braces. Her aim is to give her patients the most comfortable treatment with the best cosmetic result. Francesca is passionate about her job and loves providing her patients with beautiful, natural smiles. Away from her professional live Francesca likes spending time with her two children, walking her dog and doing yoga.

If you have breathing difficulties or problems sleeping, help is at hand from Roseneath Dental Care.

Our mouths provide a gateway to our airway and our teeth act like pillars keeping the upper and lower jaws apart, leaving room for our tongue as well as maintaining a vital breathing space. Tooth loss and a loss of tooth height leads to a reduction in the airway area. Similarly, people with small jaws have a smaller airway. Humans with larger airways tend to breathe better and often perform better athletically.

During a child’s development the jaws grow downwards and forwards to form the lower half of the face. Breast-feeding promotes good downward and forward jaw growth during a critical period, enlarging the room for the tongue and airway. Backward forces on the jaws or teeth prevent proper jaw growth, and can decrease the size of the jaws and the airway. Backward constricting forces caused by bottle and pacifier sucking can cause dental malocclusion and jaw deformation, which can also reduce tongue and airway space.

Nasal breathing is now known to be vital to good health. Mouth breathing has various causes. Bottle feeding and pacifier use prevent proper jaw growth and promotes mouth breathing. Enlarged tonsils and adenoids can cause airway obstruction leading to mouth breathing. If the cause of the airway obstruction is the enlarged tonsils or adenoids, their removal will lead to a reversal of the condition. If narrow jaws and an incorrect bite is the problem then functional jaw orthopaedics is the solution. Simple removable braces gently expand the child’s jaws correcting the bite and thereby increasing the airway space.

In adults, snoring may signal a dangerously blocked airway. During sleep, gravity can pull the lower jaw, tongue and other tissues into the throat. This makes a unique choking sound – snoring. Research shows snorers have low night oxygen and this can cause hypertension and the likelihood of morning heart attack.

Fortunately a simple solution is also available. We now have dental devices available that are worn at night to help maintain the airway. They hold the lower jaw forward and thereby keep the airway open.
